cache: Arc<DashMap<String, Vec<PushSubscriptionRecord>>>

In-memory cache: wallet -> subscriptions. Wallets with no subscriptions are cached as empty vecs. Bounded by CACHE_MAX_ENTRIES to prevent unbounded memory growth.

job_tx: Sender<PushJob>

Sender half of the push job channel. The background task owns the receiver.
